At the Southwest Montana Community Health Center, we offer and administer a variety of programs to assist people in our communities.

Health Insurance Help

At SWMTCHC, we offer assistance for our patients and the community at large in obtaining health insurance. For those under 65 who are not disabled, that includes Medicaid and the Health Insurance Marketplace for those who earn too much for Medicaid. In both of cases, we can assist you in creating an account and submitting an application. For the Marketplace, we can also provide you with information to help you decide which plan may be the best for you and your family.

For those who have been disabled for two years or more and everyone over 65, we are able to assist with the transition to Medicare, general education about Medicare and enrolling in both Advantage Plans and Part D Plans for prescription coverage. We can help you navigate and understand the complexities of Medicare, including financial assistance and even Medicaid for those who qualify. We can also provide information about working and maintaining work insurance after 65.

Healthcare for the Homeless

The clinic Healthcare for the Homeless program provides medical and supportive services for people experiencing homelessness in our area.

We provide preventative healthcare, acute/chronic illness care, mental health and substance use services, dental care and prescription assistance. We also work closely with community partners to help homeless people access needed housing and employment programs.
