COMMUNITY
For more than 35 years, SWMTCHC has been an integral part of the healthcare landscape, growing with each passing year by adding sites and services that you and your family need. We serve nearly 14,000 patients each year including those who are insured, underinsured, or uninsured and may qualify for our sliding fee scale.
WELLNESS
We believe that healthcare is a basic human right. We work hard every single day to provide primary medical, dental, pharmacy, and behavioral health services and care management to every person in our region.
